Use this form to request four weeks of general advertisement (events, businesses, announcements, and occupational therapy job postings). Please complete a new form for each advertisement you want displayed. The advertisement fee includes one post per week for four weeks on our Facebook and Instagram social media accounts. 

All advertisements are subject to approval by the AKOTA Board Members. Please allow one week to post your advertisement after Board approval and payment are received. Requests pertinent to the mission of AKOTA are more likely to be approved than others.
